Please review the following list of documents and submit all that apply to you and your household:

• Pay Stubs for the last 60 days for ALL adult household members
• Proof of Other income such as SSI/SSDI, child support, alimony, and/or self-employment
• Checking/Savings Bank Account Statements for the most recent 2 months (complete statements with all pages)
• Most Recent 2 years of Federal Tax Returns (all pages)
• ALL W-2’s or 1099’s for most recent 2 years
• Divorce Decree
• Bankruptcy Documentation if filed in the past 7 years
  Student Loan Repayment Information
